Members Only Reimbursement Form (Login Required.) Prior authorization from the Board of the Guild of Berkshire Artists is required for any expense along with a sales receipt, a description of the item and its use. All reimbursement forms must be submitted within three weeks of when the expense is incurred. See form for details.
